Dear Parent/Guardian,

I want to give you some information that I have regarding a state funded health insurance program, KidCare.  This program provides coverage to children and pregnant women and helps in paying premiums of employer-sponsored or private insurance plans.

To be eligible a child must be an Illinois resident and U.S. citizen or qualified legal immigrant and meet income requirements.  Any pregnant woman who is an Illinois resident and meets the income requirements is eligible.

Income requirements vary by KidCare Plan and are based on family size.  KidCare services are available at no cost or at low cost depending on your income.  There are several levels of coverage based on the family income.

Services for kids include doctor and nursing care, immunizations and preventive care, hospital and clinic care, laboratory tests and x-rays, prescription drugs, medical equipment and supplies, medical transportation, dental care, eye care, psychiatric care, podiatry, chiropractic care, physical therapy mental health and substance abuse services.  Pregnant women receive pre-natal care and other medical services.

Families who have employer-sponsored or private insurance can receive help from KidCare to pay their insurance premiums if their family income falls between certain monthly income standards.  KidCare participants enrolled in employer or private plans receive the services provided by those plans.

To find out more information you can call me at 747-3117, ext. 2111 or KidCare at 1-866-468-7543 (TTY:  1-877-204-1012).  Thank You for your time and have a good day.
