Dear Parent/Guardian of All Incoming Kindergarten, Second and Sixth Grade Students,

According to Public Act: 93-946 all students in kindergarten, second and sixth grades are required to have a dental examination. This bill takes effect July 1, 2005. This means that your child will need a dental exam on file by January 1, 2006. If you have been to the dentist recently check with your dentist because they may complete the form according to your child’s last exam.
